Who is the transgender patient? How do we think about therapeutic work with this population? How does our work with this population impact our identity as clinicians?

This talk is an overview of transgender patients presenting in a public health setting.  Transgender and Gender non-conforming patients face harassment and discrimination on a daily basis.  Negative outcomes for this population in health care settings are common.  As therapists, sensitivity and cultural humility are essential aspects of what we bring to the treatment.   This talk will also describe some basic terms clinicians need to know when working with the gender diverse patient.  It will focus on common themes in the treatment: weighing the harm of not treating against the potential harms of surgery and medication treatments, the meaning and importance of informed consent, and gender identity across a lifespan.  As therapists we may need to advocate for our patients with other healthcare providers and educate them about the needs and rights of this vulnerable population.  

Maria Porch, LCSW is a psychotherapist in private practice in Los Altos, CA with 17 years of clinical experience in the public sector.  She worked for 12 years at Tom Waddell Health Center in the Transgender Clinic providing assessment, intake, crisis management and counseling.  Maria is currently enrolled in the PAPPTP program through SFCP.  For more information, visit her website at:


Course Objectives:

  1. Participants will be able to articulate the societal challenges transgender and gender non-conforming individuals face in their daily lives.
  2. Participants will learn a basic language of terms that is essential to know when working with a trans gender and gender non-conforming population.
  3. Participants will be better able to manage counter transference reactions that are common in the treatment setting with trans gender and gender non-conforming patients.
